Ghost recovery: CC orphan. 오늘 만든 OPS-0507-CHAIN-ORCH-INT-STRIP-FIX의 root cause.
Bug: scripts/create_ticket.sh L50-63
- handoff JSON에 stage_to 키 누락
- to=CoS (잘못된 값) → bots_registry.json에 없는 bot
- chain_orchestrator가 받아 stage_to 정수 처리 실패 (별도 P0에서 fix)

Sub-tasks:
- S01 (BE): scripts/create_ticket.sh L50-63 — stage_to:"requirements" 추가 + to=Ops로 변경
- S02 (QA): 신규 ticket → handoff JSON 5필드 검증 + 28건 quarantined 재발행
